Branch 844 Youth Achievement Award Nomination Form
Deadline For Nomination: April 1st

-Rules and Regulations-

Purpose:
Branch 844 Los Angeles, CA, voted to establish a “Youth Achievement Award” at their December 2023 meeting in an effort to set incentives for the contributions of youth and to enhance Slovak Heritage awareness. The contest is open to all Branch 844 Members 18 years of age and under.

Rationale:
The award is directed at youth who do outstanding fraternal work in these three areas:
1. School work and sports
2. In their community or Branch
3. In their church

Nomination:
Youth Members can be nominated by an adult Branch Member. A document detailing the qualifications of the youth Member being nominated must be attached to this form and submitted to the Branch 844 office along with a min. 500-word essay about “Why is your Slovak heritage important to you?”. A youth Member may be nominated each year up to and including their eighteenth birthday. Nomination forms are available through the Branch 844 office: slovakamericancenter@gmail.com

Deadline:
Nominations must be sent to the Branch 844 office by April 1st of each year.

Judging:
An independent judge, appointed by Branch 844 President, will review all nominations and decide the winner. Branch 844 President, or an appointee, shall be the final authority on all matters involving this fraternal benefit.

Award:
The following awards will be given out to the top three nominees: $500 First Place, $300 Second Place, $100 Third Place.

The following must be signed by the nominating Member:

I wish to nominate _______________________________ to receive the Branch 844 “Youth Achievement Award” for outstanding service in their school work, community, church, local Branch, and min. 500-word essay about “Why is your Slovak heritage important to you?”. I believe that this individual should be awarded the “Youth Achievement Award” due to their service in above categories.

IMPORTANT: SUPPORTING DOCUMENTS HIGHLIGHTING THE NOMINEE MUST BE ATTACHED AND SUBMITTED TO THE BRANCH 844 OFFICE INCLUDING A RECENT PHOTOGRAPH.
